Gift from Google maps
The annual gift was a Google Local Guides T Shirt. And all Local Guides who reached Level 4 (ie, more than 200 points) by Dec 2'15 were eligible for the annual gift
As of today, I've close to 6k points and maxed out :D

There is nothing like an annual gift. Google incentives are ever changing. Forget annual gift, you don't even know what you'll get after reaching level 10. Google is still chalking out the plans for local guide program. So, let's wait for something good.
Update - There is no announcement from Google about any incentive plan as if today(October 2020)
